Episode Details

Most chiropractors are gonna wake up one day, decide they're done, and try to sell the practice. Then they're gonna find out what it's actually worth. Which is not a lot. Not because the clinical work isn't gold. Because the whole business was built around one doctor being in the building every day, and that doesn't transfer.

Dr. Kris Arnold hears this every single month. He's the incoming president of the Pennsylvania Chiropractic Association, so his phone is the one that rings when a doc says "hey, I wanna retire next month, know anyone who'd buy this?" One month. That's the timeline. The answer is almost always no. Or if it's yes, it's for pennies on the dollar.

Kris runs Live Well Chiropractic in Bedford County, Pennsylvania. Rural. 50,000 people in the entire county, and he'll happily point out there are three times more cows than people. 25 years in. 18 team members. Four docs, five as of the day we recorded. An 8,000 sq ft building in the middle of farmland doing multimillion-dollar revenue. And 35 to 36% of that revenue is now stuff that isn't a chiropractic adjustment. Functional nutrition, laser, shockwave, massage. Chiropractic is still the sun of the solar system, everything else orbits.

The whole conversation is about how Kris is NOT gonna be that doc calling the PCA in a panic. He works three clinical afternoons a week, the rest is CEO work. Runs the practice on EOS/Traction. Level 10 Meetings professionalized everything. The five-year plan is to keep ownership and step fully off the floor. He's not planning to sell. He's building the machine so he doesn't have to.

We also get into the associate arc that took him five hires and 25 years to work out. First was a train wreck. Second (Dr. Jason) almost broke because Kris was hiring a mini-me. A mentor slapped him around and told him "you don't need another you, you need someone different." Now Jason's been there 15 years. Third was a 30-year vet who couldn't be resurrected. Fourth wanted his own thing. Fifth (Dr. Randy) works. Sixth got hired the day of recording. Kris uses Chiro Matchmakers (Allen Miner (Ep 42)'s shop) to place them.

The rural blue ocean is real. 25 years ago, the five other chiros in the county were doing one-doc-one-CA setups with zero marketing. Kris ran newspaper ads first. Facebook ads first. Twenty-five years later, seven chiros in the county and only two do any social media at all. The ocean is still blue.

And the state association thing Kris drives hard: 18-19% of PA chiros are members. PT, OT, MD, veterinary associations are all around 80%. Chiro is fighting insurance battles and legislative fights on a shoestring budget for docs who aren't even members. Kris calls the association the Secret Service of the profession. Invisible. Protecting the guy who has no clue they exist.

If your practice only works when you're in it, you're within 15 years of walking away, or you're paying zero attention to your state association, this is the wake-up call.

Key Takeaways

  • Most Practices Are Worth Almost Nothing on Exit. Kris takes the call every month: "I wanna retire next month, know anyone who wants to buy this?" One month notice. Buyer's basically paying for files and a CA. Build to sell even if you don't plan to (which is exactly the thesis Stu Bernsen (Ep 31) opened up on the show).
  • From Practitioner to CEO via EOS/Traction. Gino Wickman's Traction and Level 10 Meetings is what Kris credits with pulling him out of head-down chiro mode. Three clinical afternoons a week, the rest is business. Team of 18. Four docs. Real vacations that don't blow up the practice.
  • The Rural Blue Ocean Is Real. Bedford County, PA. 50,000 people. Three times more cows than people. Seven chiros in the whole county and only two do any social media. If you're picking a market to open in, don't ignore rural.
  • Chiropractic Is the Sun of the Solar System. Every other modality (functional nutrition, laser, shockwave, massage) orbits chiropractic. Add them before chiropractic is rock-solid and you dilute. Add them after and they become spinoffs that drive their own new patients. His non-chiro services are now 35-36% of revenue.
  • Five Associate Hires to Get One Right. First was a train wreck. Second almost broke because Kris was hiring a mini-me. Third was a 30-year vet who couldn't be retrained. Fourth wanted his own thing. Fifth (Dr. Randy) is the model. Sixth got hired the day of the recording. Complementary strengths, not carbon copies. Pay for a working-week trial before you sign the contract.
  • 65-70% of New Patients from Referrals, All on Systems. Ask on day one with a discounted referral card. Four themed months a year with charity-tied referral drives. Re-exam re-report as the natural moment to ask. Systems first, incentives second. Return patient sequences bring 45-50 back a month, roughly 10 recommit to a full care plan.
  • State Association = Secret Service of Chiropractic. Only 18-19% of PA chiros are members. Compare that to 80% membership in PT, OT, MD, DO, veterinary. 3-to-1 minimum ROI, 15-to-1 if you use everything they offer. If you're not a member, the ones who are have been fighting your insurance and legislative battles while you freeload.
